Wegovy pill vs. Foundayo: the two GLP-1 weight-loss tablets compared

The Wegovy pill is oral semaglutide 25 mg, approved on 22 December 2025, and must be taken fasting with a small sip of water. Foundayo is orforglipron, approved on 1 April 2026, and can be taken any time with or without food. Label data show larger average weight loss with the Wegovy pill; no trial has compared the two directly.

The basic difference

Both tablets activate the GLP-1 receptor once a day, but they are built very differently. Semaglutide is a peptide that stomach enzymes would normally destroy, so the Wegovy tablet carries an absorption enhancer, salcaprozate sodium (SNAC), and only works if it reaches an empty stomach: one tablet in the morning with up to four ounces of plain water, then at least 30 minutes before food, other drinks or other pills. Orforglipron is a small non-peptide molecule that survives digestion on its own, so the Foundayo label allows it with or without food at any time of day. Titration differs too. The Wegovy tablet moves 1.5 mg, 4 mg, 9 mg and then 25 mg at 30-day steps. Foundayo starts at 0.8 mg, moves to 2.5 mg and 5.5 mg after at least 30 days each, and can then rise to 9 mg, 14.5 mg or a 17.2 mg maximum. Only the Wegovy tablet also carries a cardiovascular risk-reduction indication.

What the head-to-head trial found

No trial has tested the Wegovy pill against Foundayo. Each label reports its own placebo-controlled study, and both use intention-to-treat figures. In Study 7 (OASIS 4), 307 adults without diabetes took oral semaglutide 25 mg or placebo for 64 weeks; weight fell 13.6% against 2.4%, and 27.9% of the semaglutide group lost at least 20% of body weight, against 3.1% on placebo. In the Foundayo label's Trial 1 (ATTAIN-1), 3,127 adults without diabetes were followed for 72 weeks; weight fell 7.4% on 5.5 mg, 8.3% on 9 mg and 11.1% on 17.2 mg, against 2.1% on placebo. In adults with type 2 diabetes (Trial 2), the same doses reached 5.1%, 7.0% and 9.6%. There is one head-to-head trial, ACHIEVE-3 (The Lancet, 2026), but it compared orforglipron with Rybelsus-strength oral semaglutide, 7 mg and 14 mg, in 1,698 adults with type 2 diabetes. Orforglipron lowered HbA1c by 1.71 to 1.91 points against 1.23 to 1.47 for semaglutide at 52 weeks. It says nothing about oral semaglutide at the 25 mg Wegovy dose.

Side effects

Both tablets share the GLP-1 pattern of nausea, vomiting, diarrhea and constipation, worst while doses are rising. The Wegovy label says the tablet's common reactions matched Wegovy 2.4 mg injection in type and frequency, where nausea reached 44%, diarrhea 30% and vomiting 24%; 6.9% on the 25 mg tablet stopped for adverse reactions against 5.9% on placebo, and 5% reported dysesthesia against none on placebo. Foundayo's label pools its two weight trials: nausea 26-35% by dose against 10% on placebo, vomiting 13-24%, constipation 20-27% and diarrhea 21-25%, with 8% stopping for adverse reactions against 3%. Foundayo also raised resting heart rate by 4 to 5 beats per minute, hair loss was reported in 7% of women, and its dose is capped at 9 mg with strong CYP3A4 inhibitors. Both carry the thyroid C-cell tumor boxed warning and the MTC and MEN 2 contraindication.

Which one is right for you?

The Wegovy pill fits someone who wants the larger label weight-loss figure from a tablet, already has a steady morning routine, and values the cardiovascular indication. Its weak point is the ritual: an empty stomach, four ounces of water at most, and a 30-minute wait before coffee, breakfast or other medicines. Anyone with other medicines fixed to early morning should plan around that 30-minute rule with a prescriber. Foundayo fits someone for whom that ritual would fail, such as shift workers, people with irregular mornings, or anyone who wants to take a pill with lunch. The trade is a smaller average effect in its own trial, a heart-rate rise, and interaction limits with strong CYP3A4 inhibitors. Cost can separate them at the start: NovoCare lists the Wegovy tablet at $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg (NovoCare Wegovy price guide, April 2026 edition, checked 28 Sep 2026), and Temi lists Foundayo at $149 a month for 0.8 mg (usetemi.com/medications/foundayo, checked 28 Sep 2026). Neither suits someone with a personal or family history of medullary thyroid carcinoma or MEN 2, or someone whose goals call for an injectable's larger effect.

Frequently asked questions

What is the difference between the Wegovy pill and Foundayo?

The Wegovy pill is semaglutide, a peptide, taken as a 25 mg tablet once daily on an empty stomach with an absorption enhancer called SNAC. Foundayo is orforglipron, a small non-peptide molecule taken once daily with or without food at any time of day. Both are FDA-approved for chronic weight management in adults. The Wegovy tablet also carries a cardiovascular risk-reduction indication; Foundayo does not, according to its current label.

Which pill causes more weight loss?

On each drug's own label, the Wegovy pill: 13.6% average weight loss at 64 weeks in OASIS 4, against 11.1% at 72 weeks on Foundayo 17.2 mg in ATTAIN-1, both counting everyone randomized. The trials enrolled different people for different lengths of time, so the comparison is indirect. Individual results vary widely, and no pill guarantees any particular amount of weight loss for a given person.

Did ACHIEVE-3 compare Foundayo with the Wegovy pill?

No. ACHIEVE-3, published in The Lancet in 2026, compared orforglipron with oral semaglutide at 7 mg and 14 mg, the Rybelsus doses used for type 2 diabetes, over 52 weeks in 1,698 adults already on metformin. Orforglipron lowered HbA1c more at both doses, and more participants on orforglipron stopped treatment for adverse events, 9 to 10% against 4 to 5%. The 25 mg Wegovy dose was not part of the trial.

Can I take Foundayo with breakfast or coffee?

Yes. The Foundayo label says to take it once daily with or without food, swallowed whole and not broken, crushed or chewed, with no more than one tablet a day. The label's pharmacology section reports no clinically relevant food effect on orforglipron exposure, so there is no fasting window. That is the practical opposite of the Wegovy tablet, which needs an empty stomach and a 30-minute wait before eating or drinking anything other than water.

What happens if I take the Wegovy pill with food?

Semaglutide absorption from the tablet depends on reaching an empty stomach, and the label says to take it in the morning with up to four ounces of plain water only, then wait at least 30 minutes before food, other drinks or other oral medicines. Food and other liquids interfere with absorption, which is why the label rules them out. If a dose is missed, the label says to skip it and take the next one the following day.

How long does it take to reach the full dose of each pill?

The Wegovy tablet reaches its 25 mg maintenance dose on day 91, after 30 days each at 1.5 mg, 4 mg and 9 mg. Foundayo reaches 5.5 mg after at least 60 days and can then step up, at no less than 30-day intervals, to 9 mg, 14.5 mg and 17.2 mg, so the 17.2 mg maximum comes no earlier than about day 151. Prescribers can slow either schedule when side effects are heavy.

Do the trial doses of orforglipron match the Foundayo tablets?

Not by number. Trials such as ACHIEVE-3 describe orforglipron at 12 mg and 36 mg, while the approved Foundayo tablets run from 0.8 mg to 17.2 mg. The US Department of Veterans Affairs drug monograph for Foundayo reports that Lilly attested the 36 mg study formulation is equivalent to the 17.2 mg marketed tablet. Ask your prescriber how any trial figure you read translates to the strength you are prescribed.

Can I switch from the Wegovy pill to Foundayo?

Neither label gives a conversion between semaglutide and orforglipron, because they are different molecules. Foundayo's label starts every patient at 0.8 mg and says it should not be combined with another GLP-1 receptor agonist, so a switch normally means stopping the Wegovy tablet and titrating Foundayo from the bottom. Some weight regain during that transition is possible, so plan the timing with your prescriber.
