Foundayo vs. Zepbound: a daily GLP-1 pill against a weekly shot

Foundayo vs Zepbound is a choice between a daily tablet and a weekly injection, both from Eli Lilly and both approved for weight management. Zepbound (tirzepatide) produced larger average weight loss in its trials. Foundayo (orforglipron) needs no needle, fridge or fasting, and it is now studied as a way to maintain weight after injections.

The basic difference

Foundayo is orforglipron, approved on April 1, 2026. It is a small, non-peptide molecule that activates the GLP-1 receptor. That structure lets it work as an ordinary tablet, swallowed with or without food at any time of day, per its label. Doses start at 0.8 mg daily, then 2.5 mg and 5.5 mg, each for at least 30 days. They can then rise to 9, 14.5 or 17.2 mg. Tablets are stored at room temperature.

Zepbound is tirzepatide, a peptide that activates both GIP and GLP-1 receptors. It is injected under the skin once a week. It starts at 2.5 mg and rises by 2.5 mg every 4 weeks or more to a 5, 10 or 15 mg maintenance dose. Zepbound is also approved for obstructive sleep apnea with obesity, which Foundayo is not.

What the head-to-head trial found

No trial has compared Foundayo with Zepbound, so the numbers below come from separate studies with different participants.

Foundayo: ATTAIN-1 (NEJM 2025) enrolled 3,127 adults with obesity and no diabetes for 72 weeks. Counting everyone randomized, average weight change was −7.5%, −8.4% and −11.2% on the three doses, versus −2.1% on placebo. The top trial dose matches the 17.2 mg tablet. At that dose, 54.6% lost at least 10% of body weight, versus 12.9% on placebo. Lilly reports 12.4% for people who stayed on the top dose. Side effects led 5.3% to 10.3% of orforglipron users to stop, versus 2.7% on placebo.

Zepbound: SURMOUNT-1 (NEJM 2022) also ran 72 weeks and reported −15.0%, −19.5% and −20.9% at 5, 10 and 15 mg, against −3.1% on placebo.

Switching evidence. ATTAIN-MAINTAIN (Nature Medicine, 2026) moved people from tirzepatide or semaglutide to orforglipron or placebo for 52 weeks. People coming off tirzepatide kept an estimated 74.7% of their weight reduction on orforglipron, versus 49.2% on placebo. For people coming off semaglutide, the figures were 79.3% versus 37.6%. There was no arm that stayed on the injection, so it cannot show how the pill compares with continuing Zepbound.

Side effects

Stomach effects dominate both, and they are most common during dose increases. At the top doses, compared with placebo in each program:

  • Nausea: Foundayo 17.2 mg 35% vs 10%; Zepbound 15 mg 28% vs 8%
  • Vomiting: 24% vs 4%; 13% vs 2%
  • Diarrhea: 25% vs 11%; 23% vs 8%
  • Constipation: 24% vs 9%; 11% vs 5%

About 10% stopped Foundayo 17.2 mg because of side effects, versus 6.7% on Zepbound 15 mg, per the two labels.

Interactions differ. Foundayo is capped at 9 mg with strong CYP3A4 inhibitors, such as clarithromycin. Simvastatin should not exceed 20 mg. Birth-control pills need backup for 30 days after starting and after each increase; Zepbound's label says 4 weeks. Both carry thyroid tumor boxed warnings. Orforglipron did not cause tumors in rodents, but the label keeps the warning and the MTC and MEN2 contraindication.

Which one is right for you?

Zepbound may be the better fit if you want the larger average weight effect, are comfortable injecting once a week, and have fridge space. It is also the only one of the two approved for obstructive sleep apnea with obesity.

Foundayo may be the better fit if needles are the barrier or a fixed weekly day does not suit you. It also fits if you travel often, since it does not need refrigeration. It may suit people who have already lost weight on an injection and want an oral way to maintain it. ATTAIN-MAINTAIN supports that use, though not against continuing the injection.

Frequently asked questions

Can I switch from Zepbound to Foundayo to keep weight off?

That is the question ATTAIN-MAINTAIN tested. People who had lost weight on tirzepatide in SURMOUNT-5 were moved to orforglipron or placebo for 52 weeks. On orforglipron they kept about three-quarters of their weight reduction, compared with about half on placebo. Some regain was still typical. The trial did not include a group that stayed on tirzepatide, so it cannot say whether switching beats continuing.

Do I have to take Foundayo on an empty stomach?

No. The label says to take it once daily with or without food, at any time of day, and to swallow it whole without crushing or chewing. That sets it apart from oral semaglutide (the Wegovy pill and Rybelsus). Those require an empty stomach, no more than 4 ounces of water and a 30-minute wait before eating. Take no more than one Foundayo tablet a day.

What if I miss a dose of Foundayo or Zepbound?

For Foundayo, the label says to take a missed tablet as soon as possible, but never two in the same day. If you miss 7 or more days in a row, the dose ladder restarts at a lower step. For Zepbound, a missed shot can be given within 4 days (96 hours). After that, skip it and inject on your usual day. Doses must stay at least 3 days apart.

Does Foundayo interact with other medicines?

More than Zepbound does. The Foundayo label limits the dose to 9 mg with strong CYP3A4 inhibitors such as clarithromycin, and says to avoid those that also block OATP1B, such as ritonavir. It advises avoiding strong CYP3A4 inducers such as carbamazepine, and keeping simvastatin at 20 mg or less. Both drugs slow stomach emptying, which can change how other pills absorb. Is Foundayo safe if I have had thyroid problems?

It depends on the problem. Foundayo, like Zepbound, is contraindicated if you or a family member has had medullary thyroid carcinoma, or if you have MEN2. Its label notes that orforglipron did not cause thyroid tumors in rodents, but the warning stays because human relevance is unknown. Common conditions such as hypothyroidism are not listed as contraindications. Report any neck lump, hoarseness or trouble swallowing.

Does Foundayo need to be refrigerated?

No. The Foundayo label says to store the tablets at room temperature, 68°F to 77°F, in the original bottle to protect them from light. Zepbound pens and vials are normally kept in the refrigerator, with limited time allowed at room temperature per its label. For travel, a bottle of tablets is simpler than packing injections, needles and a sharps container.

Is Foundayo approved for type 2 diabetes?

Not yet. As of September 2026 its label covers weight management only. That includes adults with type 2 diabetes who also have obesity or overweight, and one of its two pivotal trials enrolled that group. Lilly reported diabetes trial results in June 2026, including a win over oral semaglutide on HbA1c in ACHIEVE-3, and said it planned to file for that use.

Can I take Foundayo and Zepbound together?

No. The Foundayo label says it should not be used with another GLP-1 receptor agonist, and the Zepbound label says the same about any GLP-1 drug. Taking both would stack similar effects on the gut and raise side-effect risk, with no evidence of added benefit. If you are changing from one to the other, your prescriber will set the timing.
