Telehealth Consent

BY CHECKING THE BOX INDICATING YOUR AGREEMENT TO THIS TELEHEALTH CONSENT, OR BY OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, ACCEPTED, AND AGREE TO BE BOUND BY THIS CONSENT. IF YOU DO NOT AGREE TO THIS CONSENT, DO NOT USE THE SERVICE.

IF YOU ARE EXPERIENCING A LIFE-THREATENING SITUATION SUCH AS CONTEMPLATING SUICIDE, CALL 911 OR THE 988 SUICIDE & CRISIS LIFELINE AT 988.

1. What This Consent Covers

By providing your consent, you agree to receive asynchronous telehealth services from Temi Medical Group, through the Temi platform operated by Self Systems, Inc.

This consent applies to asynchronous telehealth consultations and ongoing care for non-controlled medications, as described below.

2. Who Provides Your Care

Self Systems, Inc. operates the Temi technology platform. Self Systems, Inc. does not provide clinical care and does not practice medicine. Self Systems, Inc. does not control or interfere with the practice of medicine by Temi Medical Group or its physicians.

Temi Medical Group is the professional medical entities through which licensed physicians render care to patients in the states where Temi operates. The professional medical entity that contracts with you depends on the state in which you receive services. Temi Medical Group is not owned or controlled by Self Systems, Inc. Self Systems, Inc. provides administrative and technology services to Temi Medical Group under a management services arrangement and does not control its clinical practice.

Your doctor-patient relationship is with Temi Medical Group and its treating physicians, not with Self Systems, Inc.

Each treating physician exercises independent clinical judgment. A physician may decline any prescription request based on their professional assessment of your health information.

3. Nature of Telehealth Services

Temi Medical Group provides asynchronous telemedicine services for non-controlled medications. The process works as follows:

  • You submit a consultation request through the Temi platform, including your health information and the medication you are requesting.
  • A licensed physician reviews your request asynchronously (not in real-time).
  • If the physician determines that a prescription is clinically appropriate, the prescription is transmitted electronically to your pharmacy of choice or, where applicable, to a Temi partner pharmacy for fulfillment.
  • If the physician determines that a prescription is not appropriate, your request will be declined. Refund handling for declined requests is described in the Terms of Service.

Temi is not a substitute for in-person medical care, emergency services, or ongoing primary care. If you have a condition that requires in-person evaluation, your physician may recommend that you seek in-person care.

Compounded medications. Some medications prescribed through Temi, including compounded semaglutide and compounded tirzepatide, are compounded by a licensed pharmacy and are not FDA-approved finished drug products. Their use for weight management may be off-label. Your physician will discuss the risks, benefits, and alternatives with you, and your agreement to proceed is your informed consent to treatment with a compounded medication.

Anticipated benefits. Telehealth may offer greater convenience, improved access to care, and reduced travel and wait times. These benefits cannot be guaranteed, and your physician cannot promise any particular clinical outcome.

4. Risks and Limitations of Telehealth

You should be aware of the following risks and limitations:

  • Limitations of asynchronous care. Telehealth consultations conducted asynchronously have limitations compared to in-person examinations. The physician will make clinical decisions based solely on the information you provide, without the benefit of a physical examination.
  • Incomplete information. If the information you provide is insufficient, the physician may be unable to make a clinical determination and may decline your request.
  • Technology risks. The platform depends on technology that may experience disruptions, including connectivity issues, software errors, or delays in transmission.
  • Privacy risks. Although we use encryption and security measures to protect your information, electronic communications carry inherent privacy risks.
  • Referral to in-person care. The physician may determine that your condition is not appropriate for telehealth and may decline to prescribe, recommending that you seek in-person evaluation instead.

5. Your Rights

  • You may seek in-person care from a provider of your choice at any time.
  • You may withdraw this consent at any time by contacting us at [email protected] or +1 (855) 554-3030, without prejudice to any future care or treatment, and without risking the loss or withdrawal of any health benefits to which you are otherwise entitled. Withdrawal of consent will not affect any services already provided.
  • This consent is voluntary. Providing or withholding consent for telehealth services through Temi is not a condition of receiving in-person care from any other provider.

6. No Controlled Substances

Temi Medical Group does not prescribe controlled substances through the Temi platform. Requests for controlled substances will be declined.

7. No Guarantee of Prescription

Submitting a consultation request does not guarantee that a prescription will be issued. The treating physician may decline your request for any clinical reason. If your request is declined, no medication charge is made; a non-member's prescription fee for that request is refunded and a member's membership is unaffected, as described in the Terms of Service.

8. Not for Emergencies

TEMI MEDICAL GROUP DOES NOT PROVIDE EMERGENCY MEDICAL CARE. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO YOUR NEAREST EMERGENCY ROOM IMMEDIATELY. IF YOU ARE EXPERIENCING A LIFE-THREATENING SITUATION SUCH AS CONTEMPLATING SUICIDE, CALL 911 OR THE 988 SUICIDE & CRISIS LIFELINE AT 988.

9. Patient Acknowledgment

By providing your consent, you acknowledge and agree to the following:

  • I have read and understand this Telehealth Consent.
  • I voluntarily consent to receive telehealth services from Temi Medical Group through the Temi platform.
  • I understand that this consent covers asynchronous telehealth services for non-controlled medications, including initial weight-loss consultations and ongoing care, and treatment with compounded medications that are not FDA-approved finished products.
  • I understand that submitting a request does not guarantee that a prescription will be issued.
  • I understand that any increase in my medication dose is authorized only after I respond to my physician's follow-up, and that if I do not reply my next dose will not be authorized.
  • I agree to provide accurate and complete health information.
  • I understand that I may withdraw this consent at any time.

10. In-App Messaging and Dose Changes

The Temi patient dashboard includes an in-app messaging channel between you and your treating physician for non-urgent clinical questions related to your active treatment. This channel is not over SMS.

  • Non-urgent only. The messaging channel is for non-urgent questions about your active treatment. It is not for new diagnoses or conditions outside your active treatment.
  • No guaranteed response time. Your physician may not respond promptly to messages sent through this channel. Time-sensitive concerns should be addressed through your local emergency services or in-person urgent care, not through Temi.
  • Dose changes require your reply. Any increase in your medication dose is authorized only after you respond to your physician's follow-up check-in. If you do not reply, your next dose will not be authorized.
  • If messaging is unavailable. If a technical problem prevents you from reaching your care team in the dashboard, call +1 (855) 554-3030.
  • Not for emergencies. If you are experiencing a medical emergency, do not message Temi. Call 911 or go to your nearest emergency room.
  • Adverse effects. Significant adverse effects from a medication you are receiving through Temi should be reported promptly through this channel and may also require in-person evaluation. Severe symptoms should not wait for a message reply; seek emergency care.

11. State Medical Board Complaints

If you have a concern about a medical professional treating you through Temi, you may contact the medical board in your state regarding your concerns.
