What you pay with insurance

For type 2 diabetes, Lilly says Mounjaro may be covered by some insurance plans, and your share depends on your formulary tier, deductible and whether a prior authorization is approved.

Lilly's own estimates, from IQVIA data for October 2024 to September 2025 (Lilly Mounjaro FAQ, checked 5 Oct 2026):

  • Commercial plans: almost 9 in 10 people pay $0 to $50 per 28-day supply; the rest average $258.
  • Medicare Part D: about 8 in 10 pay $0 to $50; the rest average about $262. Lilly notes a $2,400 yearly out-of-pocket cap for Part D in 2027.
  • Medicaid: about $5 to $11 a month on average.

If Mounjaro is prescribed for weight loss alone, expect a denial: it is not approved for that, and the savings card requires an approved use. Before filling, ask your insurer three things: is Mounjaro on the formulary, which tier is it, and is prior authorization required?

Cost without insurance

With no insurance, the starting point is the $1,112.16 list price per four-pen fill (Lilly pricing page, checked 5 Oct 2026). Lilly's FAQ says to expect the full retail price without prescription coverage, plus any taxes and fees, and Lilly notes that pharmacy prices can differ from list.

The main lower route is LillyDirect self-pay, which lists every single-dose pen strength from $499 a month (LillyDirect, checked 5 Oct 2026). Your prescriber sends the prescription to LillyDirect Pharmacy, and a partner pharmacy delivers or arranges retail pickup.

The Mounjaro Savings Card does not help here: both versions require commercial drug insurance. Discount cards may lower a retail cash quote, but they are not insurance. Ask the pharmacy for its cash price on your exact strength before filling.

Savings programs

The Mounjaro Single-Dose Pen Savings Card has two tiers (Lilly savings terms, checked 5 Oct 2026):

  • Plan covers Mounjaro: pay as little as $25 for a 1-, 2- or 3-month fill. Savings cap at $150 per month, $450 per 3-month fill and $1,950 a year.
  • Plan does not cover Mounjaro: pay as low as $499 per 28-day fill. Savings cap at $647 a month and $8,411 a year.

Both require commercial drug insurance, age 18 or older, US or Puerto Rico residence and a prescription for an FDA-approved use. Anyone on Medicare, Medicaid, TRICARE, VA or DoD coverage is excluded. The card covers single-dose pens only, not the KwikPen or vials, allows 13 fills a year and ends 31 December 2026.

Cost over a year

List price: $1,112.16 × 13 four-week fills = $14,458.08 a year.

Uncovered commercial plan with the card: $499 × 13 = $6,487 a year, assuming the card's $647 monthly cap covers the rest.

LillyDirect self-pay: from $499 × 13 = $6,487 a year.

Covered plan with the card: the card pays up to $150 a fill and $1,950 a year. A $175 copay drops to $25 for all 13 fills, using exactly the $1,950 cap; a higher copay leaves you paying the difference.

Compounded tirzepatide at Temi (not FDA-approved): at 5 mg a week, 20 mg × $12.35 per mg = $247.00, plus $39 = $286.00 per 4 weeks in California. Vials come in 8.5 mg, so real orders round up. Temi Care at $449 a year covers the $39 fee, so it pays back after 12 compounded orders ($468).

Lower-cost options

Each option below is a different product or route, not a cheaper Mounjaro:

  • Zepbound (tirzepatide for weight management): LillyDirect KwikPen from $299 to $449 a month with on-time refills (LillyDirect, checked 5 Oct 2026). Only fits if you meet Zepbound's label.
  • Foundayo (orforglipron, a daily pill): $149 per 30-day supply of the 0.8 mg starting dose through LillyDirect (usetemi.com/medications/foundayo, checked 5 Oct 2026). Temi physicians can prescribe it; LillyDirect dispenses it.
  • Compounded tirzepatide: Temi in California lists it from $104.99 per 8.5 mg vial, plus $39 (usetemi.com/medications/compounded-tirzepatide-injectable, checked 5 Oct 2026). This is not FDA-approved, is not a generic, and is not the same product as Mounjaro.

Frequently asked questions

Why does Mounjaro cost the same at every dose?

Lilly sets one list price, $1,112.16 per fill of four pens, whichever strength is in them (Lilly pricing page, checked 5 Oct 2026). LillyDirect's self-pay price is also the same, from $499 a month, across 2.5 mg to 15 mg (LillyDirect, checked 5 Oct 2026). So moving up a dose does not change the list or self-pay cost, although your insurer's tier can.

Can I use the Mounjaro savings card with Medicare?

No. The card's terms exclude anyone enrolled in Medicare, including Part D and Medicare Advantage, as well as Medicaid, TRICARE, VA and DoD programs (Lilly savings terms, checked 5 Oct 2026). Lilly instead points Medicare patients to the Medicare Prescription Payment Plan, which spreads covered costs into monthly payments, and to Extra Help for those who qualify.

Does the Mounjaro savings card work for the KwikPen?

Not under the current terms. The Mounjaro Single-Dose Pen Savings Card lists only the six single-dose pen NDCs and says other methods of administration are not eligible (Lilly savings terms, checked 5 Oct 2026). If your prescription is for a KwikPen or vial, ask your pharmacist what your plan charges for it before filling.

How much is a 3-month supply of Mounjaro?

At list price, three four-pen fills come to $3,336.48 (3 × $1,112.16, Lilly pricing page, checked 5 Oct 2026). With a commercial plan that covers Mounjaro, the savings card can bring an 84-day, 12-pen fill to as little as $25, with savings capped at $450 for that fill (Lilly savings terms, checked 5 Oct 2026). Whether your plan allows a 3-month fill is up to the plan.

Is Mounjaro cheaper through LillyDirect than at a pharmacy?

For people paying cash, usually. LillyDirect lists single-dose pens from $499 a month (LillyDirect, checked 5 Oct 2026), against a $1,112.16 list price that retail cash prices start from (Lilly pricing page, checked 5 Oct 2026). With good insurance coverage, a regular pharmacy and your plan's copay may cost less. Compare both quotes for your exact strength.

Does Temi sell Mounjaro?

No. Mounjaro is not sold through Temi. Temi offers compounded tirzepatide, which is not FDA-approved and is not a generic of Mounjaro, from $104.99 per 8.5 mg vial plus a $39 prescription service per order in California (usetemi.com/medications/compounded-tirzepatide-injectable and /how-it-works, checked 5 Oct 2026). A Temi physician reviews each request.
