How to use Mounjaro: single-dose pen, KwikPen and vial

Inject Mounjaro under the skin of your stomach or thigh once a week, any time of day, with or without food; someone else can use the back of your upper arm. Your steps depend on the device: the single-dose pen, the four-dose KwikPen that you prime every week, or a vial and syringe.

Before you start

The August 2026 Mounjaro label lists four presentations: a single-dose autoinjector pen, a single-dose vial (0.5 mL), a multi-dose vial and a four-dose KwikPen (0.6 mL per dose). Confirm which one you were dispensed, because the label asks for device-specific training and new training whenever the presentation changes. Tell your prescriber about any personal or family history of medullary thyroid carcinoma or MEN 2, pancreatitis, gallbladder disease, diabetic eye disease, kidney problems, pregnancy plans, or use of insulin or a sulfonylurea. If you take an oral contraceptive, the label says to switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase.

Step by step

  1. Check the medicine

    Wash your hands. Check the name, dose strength and expiry date against your prescription. The liquid should be colorless to slightly yellow, with no cloudiness or particles. Do not use Mounjaro that has been frozen, and do not use a single-dose pen that has been dropped on a hard surface, since it contains glass parts.

  2. Single-dose pen: uncap and place

    Leave the gray base cap on until you are ready. With the pen still locked, pull the cap straight off and discard it; never put it back on. Place the clear base flat on your stomach or thigh, then turn the lock ring to unlock. The Instructions for Use say you do not need to warm the pen first.

  3. Single-dose pen: press for up to 10 seconds

    Press and hold the purple button for up to 10 seconds. The first loud click means the injection started; the second loud click means it is complete. A soft click just before the second is normal. Keep the pen on the skin until that second click and check that the gray plunger shows.

  4. KwikPen: attach a needle and prime

    Wipe the red inner seal, twist on a new KwikPen-compatible needle (not included), and remove both shields, keeping the outer one. Prime before every weekly dose: turn the knob 2 clicks to the prime line, tap air to the top, push in and count slowly to 5. Repeat up to 2 more times if no drop appears.

  5. KwikPen: dial and inject

    Turn the dose knob all the way until it stops and the dose icon shows; this is your full weekly dose, so do not count clicks. Insert the needle, push the knob in until it stops and hold for a slow count of 5. Confirm the icon is still showing before you withdraw.

  6. Vial: draw up the dose

    Use a new 1 mL syringe and needle every time. Draw in air equal to your dose, push it into the vial, turn the vial upside down and draw slightly past the line: 0.5 mL for a single-dose vial or 0.6 mL for a multi-dose vial. Tap bubbles to the top and push back to the exact line.

  7. Vial: inject and hold

    Pinch the skin if your clinician told you to, insert the needle, and push the plunger all the way down. The vial Instructions for Use say to leave the needle in for at least 5 seconds so the whole dose goes in. A single-dose vial is used once, even if liquid remains.

  8. Aftercare and disposal

    If you see blood, press gauze on the spot without rubbing. Put single-dose pens, syringes and pen needles straight into a sharps container. For the KwikPen, recap with the outer shield, unscrew the needle, and store the pen with its cap on and no needle attached to avoid leaks and air entering.

Schedule

Mounjaro is injected once weekly, at any time of day, with or without meals. The label's starting dose is 2.5 mg a week. If more glucose control is needed, your prescriber may raise it in 2.5 mg steps after at least 4 weeks on each dose. The maximum is 15 mg weekly for adults and 10 mg weekly for children aged 10 and over. A KwikPen or multi-dose vial holds four weekly doses of one strength, so a dose change means a new pen or vial. If you want to change your injection day, the label requires at least 3 days (72 hours) between the two doses.

How and where to take it

You or another person can inject into your stomach area (at least 2 inches from the belly button) or the front of your thigh. The label says the back of the upper arm should be injected by another person. Rotate the injection site every week; you can stay in the same area but choose a different spot within it. The vial Instructions for Use explain why: repeated use of one spot can cause pits, thickened skin or lumps. Avoid skin that is tender, bruised, scaly, hard or scarred. With insulin, inject separately and not next to each other.

Storage

Keep all Mounjaro refrigerated at 36°F to 46°F (2°C to 8°C) in the original carton to protect it from light, and never freeze it. A single-dose pen or single-dose vial may be kept unrefrigerated at up to 86°F (30°C) for a total of 21 days, then discarded. The KwikPen and multi-dose vial follow different rules: throw them away 30 days after first use, after 30 days at room temperature, or after 4 weekly doses, whichever comes first, even if medicine remains. Store a KwikPen with no needle attached.

If you miss a dose

The Mounjaro label allows a 4-day window. If you miss a dose, take it as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and take the next dose on your regular day, then carry on weekly. The Medication Guide adds one firm limit: never take 2 doses within 3 days (72 hours) of each other. On a KwikPen, do not try to inject leftover medicine to make up a dose.

If you take too much

If you take too much Mounjaro, the Medication Guide says to call your healthcare provider or the Poison Help line at 1-800-222-1222, or go to the nearest emergency room right away. Severe nausea, vomiting and low blood sugar, especially with insulin or a sulfonylurea, are the likely problems. The label notes tirzepatide's half-life is about 5 days, so a period of observation and supportive care may be needed.

What to avoid

Never share a Mounjaro pen, KwikPen, needle or syringe, even with a new needle. Do not reuse syringes or pen needles, and do not inject Mounjaro in the same spot as another medicine. Avoid relying on oral birth control alone during the 4 weeks after starting and after each increase. Go easy on alcohol, which can add to low blood sugar with insulin or a sulfonylurea. Be careful driving if you feel dizzy or shaky, and tell anyone sedating you for a procedure that you take Mounjaro.

Frequently asked questions

How do I know the Mounjaro pen injection is finished?

With the single-dose pen, listen for two loud clicks: the first starts the injection and the second, within about 10 seconds, completes it. Then look for the gray plunger in the window. If the gray plunger is visible, the dose was delivered. If it is not, the Instructions for Use say to call Lilly at 1-800-545-5979 rather than injecting another pen.

Does the Mounjaro injection hurt, and should I warm the pen?

Most people describe a quick pinch. The single-dose pen Instructions for Use state that it is not necessary to warm the pen to room temperature before injecting, though some people find a cold injection stings more. Letting the alcohol dry before injecting and rotating sites weekly help. Report lasting pain, swelling, lumps or a rash to your prescriber, as the label lists injection-site reactions.

Is it normal to see blood or a drop after injecting?

Yes. The Instructions for Use describe a drop of liquid or blood on the skin as normal. Press a cotton ball or gauze on the spot and do not rub. A drop on the needle tip when you remove the gray base cap, or after a KwikPen dose, is also normal and does not change your dose, according to the Instructions for Use.

What if I see air bubbles?

Bubbles in the single-dose pen are normal and need no action. In a KwikPen, tapping air to the top and priming before every dose clears it. With a vial, the Instructions for Use say to tap the syringe so bubbles rise, then push the plunger up to exactly 0.5 mL or 0.6 mL before injecting, so air does not take the place of medicine.

How many units is a Mounjaro vial dose?

A single-dose vial holds 0.5 mL, and a multi-dose vial dose is 0.6 mL. On a standard U-100 insulin-type syringe, 0.5 mL lines up with the 50-unit mark and 0.6 mL with the 60-unit mark. Can I switch from the single-dose pen to the KwikPen or a vial?

Your prescriber can change the presentation, but the steps are not the same. The label asks for new training whenever it changes, because the KwikPen needs a needle and priming each week and a vial needs a syringe. Doses per device also differ: 0.5 mL in single-dose products and 0.6 mL in four-dose ones. Read the new Instructions for Use before your first dose.

How do I travel with Mounjaro?

Carry your pens or vials in your hand luggage and keep them out of hot cars. A single-dose pen or vial can go up to 21 days in total at up to 86°F, and an in-use KwikPen can stay at room temperature for its 30 days, so short trips rarely need ice packs. Take spare pen needles or syringes and a small sharps container.

Can I drink alcohol while taking Mounjaro?

The Mounjaro label has no specific alcohol ban. It does warn that low blood sugar is more likely when Mounjaro is combined with insulin or a sulfonylurea, and alcohol can lower blood sugar further. Alcohol may also worsen nausea while doses are going up. If you drink, keep it moderate and with food, and check with your prescriber about your other medicines.
