Temi vs. LillyDirect: brand Zepbound, compounded tirzepatide, or both

LillyDirect is Eli Lilly's own pharmacy: it sells self-pay Zepbound vials from $299 a month but needs a prescription from someone else [1]. Temi is a prescriber. A Temi clinician can prescribe Zepbound for LillyDirect to fill [5], or compounded tirzepatide from $104.99 a vial plus $39 [7].

The core difference

LillyDirect and Temi do different jobs, and they can be used together. LillyDirect is the manufacturer's pharmacy channel. It dispenses Lilly medicines, including Zepbound single-dose vials, which are sold only through LillyDirect and only to self-pay patients, but it does not write prescriptions; it asks your doctor to send one, and links to independent telehealth providers if you do not have a doctor [2]. Temi is the other half. Its physicians review requests online, and when branded Zepbound is clinically appropriate they can prescribe it, with LillyDirect filling it at Lilly's self-pay price [5]. Temi does not set or charge that price. Where the two actually compete is on the drug itself. LillyDirect sells only the FDA-approved product, from $299 to $449 a month depending on dose [1]. Temi also offers compounded tirzepatide, which is not an FDA-approved product, starting at $104.99 per 8.5 mg vial plus a $39 prescription service fee [7].

Where the other provider offers more

LillyDirect's strongest point is the product itself. Everything it ships is manufactured by Lilly and FDA-approved, and Zepbound's label, dosing and trial record all describe that product; compounded tirzepatide is a separate preparation that is not FDA-approved [7]. Buying from the maker also settles the question of where the drug came from. Its pricing is published per dose, fixed by Lilly, and the higher doses sit at $449 a month as long as you refill within 45 days [1]. It covers more ground than a cash-pay prescriber can: KwikPens can go through commercial insurance with a savings card, there is a Medicare GLP-1 Bridge route for eligible patients, and delivery is free, with retail pickup for select medicines [1]. If you already have a doctor willing to prescribe, LillyDirect needs no middleman at all. Temi does not compete with any of that; for the branded drug it can only be your prescriber.

Frequently asked questions

How much is Zepbound on LillyDirect?

Lilly's self-pay prices are $299 a month for 2.5 mg, $399 for 5 mg and $449 for 7.5 mg, 10 mg, 12.5 mg and 15 mg, for vials and KwikPens alike [1]. The $449 price on higher doses depends on the Zepbound Journey Program: refill within 45 days or the price rises to $499 for 7.5 mg and $699 for 10 mg and above. A month is 28 days, or four vials.

Do I need a prescription to buy from LillyDirect?

Yes. LillyDirect is a pharmacy, so a licensed prescriber has to send your prescription to LillyDirect Pharmacy [2]. If you do not have one, its site lists independent telehealth providers, noting that their quoted rates exclude medication and that Lilly does not endorse them. A Temi clinician can also prescribe Zepbound when clinically appropriate, and LillyDirect then fills it [5].

Does LillyDirect take insurance for Zepbound?

For KwikPens, yes: select insurance is accepted, and commercially insured patients with coverage may pay as little as $25 with a savings card, which excludes government beneficiaries [1]. Single-dose vials are self-pay only. Temi never bills insurance, so if your plan covers Zepbound, the insured KwikPen route through LillyDirect will almost certainly cost you less than any cash option.

Is compounded tirzepatide the same as Zepbound?

It contains the same active ingredient, but it is not the same product. Zepbound is Lilly's FDA-approved medicine. Compounded tirzepatide is prepared by a compounding pharmacy and is not FDA-approved [7]. Temi offers both routes: it can prescribe branded Zepbound for LillyDirect to fill, or compounded tirzepatide from a named pharmacy. Our compounded tirzepatide vs. Zepbound page sets out the differences in detail.

Is Temi cheaper than LillyDirect?

For the branded drug, no: a Temi prescription for Zepbound is filled by LillyDirect at Lilly's price [5]. The saving only appears if you and your clinician choose compounded tirzepatide, which starts at $104.99 per 8.5 mg vial plus a $39 service fee [7], against $299 to $449 a month for Zepbound. Vial size and weekly dose decide how long each lasts.

What happens if I miss the 45-day LillyDirect refill window?

On 7.5 mg and higher doses, Lilly's page says the regular price applies instead of the Journey Program price: $499 a month for 7.5 mg and $699 for 10 mg, 12.5 mg and 15 mg [1]. The 2.5 mg and 5 mg prices are not tied to that window. Temi's compounded prices carry no refill-timing condition, though a lapse in treatment is a conversation to have with your clinician.

Can Temi send my Zepbound prescription to LillyDirect?

Yes. Temi's Zepbound page says a Temi clinician can prescribe branded Zepbound when clinically appropriate and that it is filled through LillyDirect, with you paying LillyDirect's self-pay price [5]. Temi does not sell or dispense the brand and does not set that price. Submitting a request starts a clinician review; it does not guarantee a prescription.

Does LillyDirect sell Foundayo?

Yes. Foundayo, Lilly's once-daily oral GLP-1, is dispensed through LillyDirect at $149 a month for 0.8 mg and 2.5 mg, $199 for 5.5 mg and $299 for 9 mg and above; the 2.5 mg and 5.5 mg prices are temporarily reduced through 31 December 2026 [3]. A Temi physician can prescribe Foundayo, but Temi does not fill it through its own checkout [6].
