Rybelsus alternatives: the Ozempic pill and 8 other options, priced
The first Rybelsus alternative is its own successor: Novo Nordisk is retiring the Rybelsus name, and the same daily semaglutide now comes as the Ozempic pill in 1.5, 4 and 9 mg tablets. Beyond that, adults with type 2 diabetes can look at weekly injections, generic liraglutide, metformin or dapagliflozin.
The medication in brief
Rybelsus is oral semaglutide from Novo Nordisk, a once-daily GLP-1 tablet for blood sugar control in adults with type 2 diabetes, also labeled to lower the risk of heart attack, stroke and cardiovascular death in high-risk adults (Rybelsus and Ozempic tablets label). It is being phased out. NovoCare now states that "Rybelsus is being replaced by Ozempic pill", and an Illinois Medicaid notice of 21 May 2026 says Novo will discontinue every strength, with wholesaler shipments ending in May 2026 and stock becoming limited from July. The replacement costs $149, $199 or $299 a month self-pay for 1.5, 4 or 9 mg (NovoCare Pharmacy, checked 5 Oct 2026). So most people searching now need a new prescription for something.
Why people look for alternatives
The pharmacy cannot get Rybelsus
With shipments to wholesalers stopped in May 2026, pharmacies are working through what is left (Illinois HFS notice). A new prescription is needed for any replacement, including the Ozempic pill, which uses different strengths.
No self-pay Rybelsus price
NovoCare Pharmacy lists self-pay prices for the Ozempic pill and pen, not for Rybelsus (NovoCare Pharmacy, checked 5 Oct 2026). Cash payers end up comparing the replacement with other drugs anyway.
The morning fasting routine
Both semaglutide tablets must be taken on an empty stomach with no more than 4 oz of plain water, then nothing else for 30 minutes (Rybelsus and Ozempic tablets label). Some people would rather inject once a week.
Wanting a bigger effect on HbA1c or weight
The Ozempic pen at 1 mg lowered HbA1c by 1.6 points over 30 weeks in its monotherapy trial (Ozempic label), and Mounjaro beat injected semaglutide head to head in SURPASS-2. No trial compares either directly with Rybelsus.
Using it for weight, not diabetes
Rybelsus has no weight-management indication. The Wegovy pill (semaglutide 25 mg) and Foundayo do, which matters for both the label and insurance.
How we compared
Each option's approved uses, warnings and trial averages come from its current FDA label on DailyMed; the Rybelsus phase-out comes from NovoCare and the Illinois Medicaid provider notice. Cash prices come from the seller's own page or price list (NovoCare, Lilly, Cost Plus Drugs, Temi) and were read on 5 October 2026. Nothing here was tested by us, and no seller paid to appear.

Alternatives
Ozempic pill (semaglutide tablets 1.5, 4, 9 mg)
- Kind
- FDA-approved GLP-1
- How it differs
- The reformulated semaglutide tablet that replaces Rybelsus. Same label, same daily empty-stomach routine, different milligram strengths.
- Evidence
Rybelsus and the Ozempic pill share one prescribing information, so the PIONEER trial results and the cardiovascular indication apply to both (Rybelsus and Ozempic tablets label). The label warns the two are not substitutable milligram for milligram. The Illinois notice reports Novo's suggested conversion: Rybelsus 7 mg to Ozempic 4 mg, and 14 mg to 9 mg. Your prescriber decides the actual dose.
- Starting price
- $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg, 30 tablets, self-pay (NovoCare Pharmacy, checked 5 Oct 2026)
- Best for
- Anyone stable on Rybelsus who wants to stay on daily oral semaglutide with the fewest changes
- Tradeoffs
- Needs a new prescription; Illinois Medicaid also requires a new prior authorization. Fasting rules are unchanged. Diabetes indication only.
Ozempic pen (semaglutide injection)
- Kind
- FDA-approved GLP-1
- How it differs
- Semaglutide again, injected once a week instead of swallowed daily. No fasting window, but a needle.
- Evidence
In a 30-week monotherapy trial on the Ozempic label, HbA1c fell 1.6 points on 1 mg and 1.4 on 0.5 mg, against 0.1 on placebo, and 70% to 73% of people reached an HbA1c under 7% (Ozempic label). Those are trial averages. The pen also carries indications for cutting cardiovascular risk and kidney disease progression in some adults with type 2 diabetes.
- Starting price
- Introductory $199 a month for two fills at 0.25 or 0.5 mg, then $349 (up to 1 mg) or $499 (2 mg) (NovoCare Pharmacy, checked 5 Oct 2026)
- Best for
- People who find the daily fasting routine hard to keep and are comfortable with a weekly injection
- Tradeoffs
- Costs more per month than the low-dose pill. Same thyroid C-cell boxed warning and GI side effects. Starts with its own dose escalation, set by your prescriber.
Mounjaro (tirzepatide injection)
- Kind
- FDA-approved GLP-1
- How it differs
- A weekly GIP and GLP-1 dual agonist from Eli Lilly, approved for type 2 diabetes. A different molecule from semaglutide.
- Evidence
SURPASS-2 compared Mounjaro with semaglutide 1 mg injection for 40 weeks in adults on metformin. HbA1c dropped 2.0 to 2.3 points across Mounjaro doses versus 1.9 on semaglutide, and body weight fell 7.6 to 11.2 kg versus 5.7 kg (Mounjaro label). The comparison was with injected, not oral, semaglutide. Averages only.
- Starting price
- As low as $499 a month with the Mounjaro Savings Card if you have commercial insurance that does not cover it (Mounjaro savings page, checked 5 Oct 2026)
- Best for
- Adults with type 2 diabetes whose HbA1c or weight goals were not met on oral semaglutide
- Tradeoffs
- The $499 card needs commercial insurance; uninsured cash prices are higher. Weekly injection. Same thyroid boxed warning as semaglutide.
Generic liraglutide (Victoza equivalent)
- Kind
- FDA-approved GLP-1
- How it differs
- An older GLP-1 receptor agonist injected once a day. Available as a generic, which no form of semaglutide is.
- Evidence
In a 52-week monotherapy trial on the Victoza label, HbA1c fell 1.1 points on 1.8 mg and 0.8 on 1.2 mg, versus 0.5 on glimepiride, and weight fell 2.5 kg and 2.1 kg versus a 1.1 kg gain (Victoza label). These are averages from one trial. Like semaglutide, it carries the thyroid C-cell boxed warning.
- Starting price
- $97.80 for a box of two 3 mL pens before shipping (Cost Plus Drugs price list, checked 5 Oct 2026); how long a box lasts depends on your dose
- Best for
- Cash payers with type 2 diabetes who want a GLP-1 at a generic price and can manage a daily injection
- Tradeoffs
- Daily rather than weekly injections. Smaller HbA1c and weight effects than semaglutide in its own trials.
Metformin (generic)
- Kind
- FDA-approved other weight-loss drug
- How it differs
- Not a GLP-1 and not approved for weight loss. The long-standing first oral drug for type 2 diabetes, taken with meals.
- Evidence
Metformin's label covers blood sugar control in type 2 diabetes. In the Diabetes Prevention Program, adults with raised blood sugar who took 850 mg twice a day developed diabetes 31% less often than those on placebo over an average 2.8 years (NEJM 2002). It carries a boxed warning for lactic acidosis, and kidney function guides its use.
- Starting price
- Thirty 500 mg tablets cost $5.32, or $10.57 once standard shipping is added (Cost Plus Drugs price list, checked 5 Oct 2026)
- Best for
- Adults with type 2 diabetes for whom cost matters most, often alongside another drug
- Tradeoffs
- Little effect on weight. Stomach upset and diarrhea are common early. Usually a base drug rather than a full replacement for semaglutide.
Dapagliflozin (generic Farxiga)
- Kind
- FDA-approved other weight-loss drug
- How it differs
- An SGLT2 inhibitor tablet that lowers blood sugar by making the kidneys pass more glucose in urine. Not a GLP-1 and not a weight-loss drug.
- Evidence
In a 24-week monotherapy trial on the Farxiga label, HbA1c fell 0.9 points on 10 mg against 0.2 on placebo. The label also covers lowering the risk of kidney decline and heart failure hospitalization in adults with chronic kidney disease or heart failure. Genital yeast infections occurred in 4.8% on 10 mg versus 0.9% on placebo (Farxiga label).
- Starting price
- $7.43 for 30 tablets of 10 mg before shipping (Cost Plus Drugs price list, checked 5 Oct 2026)
- Best for
- Adults with type 2 diabetes who also have heart failure or chronic kidney disease, or who want a once-daily pill with no fasting rule
- Tradeoffs
- Smaller HbA1c drop than semaglutide. Genital infections, dehydration and a rare risk of ketoacidosis are on the label.
Wegovy pill (semaglutide 25 mg tablet)
- Kind
- FDA-approved GLP-1
- How it differs
- Daily oral semaglutide at a higher dose, approved for chronic weight management rather than blood sugar control.
- Evidence
In a 64-week trial of adults with obesity or overweight and no diabetes (OASIS 4, Study 7 on the label), body weight fell 13.6% on the 25 mg tablet against 2.4% on placebo, and 59.8% lost at least 10% (Wegovy label). These are averages from one trial.
- Starting price
- $149 a month for 1.5 mg, rising to $299 for the 9 mg and 25 mg tablets, self-pay (NovoCare Wegovy price page, checked 5 Oct 2026)
- Best for
- People who took Rybelsus mainly for weight and want an oral semaglutide labeled for that purpose
- Tradeoffs
- Not a diabetes drug on its label. The same fasting routine applies. Coverage for weight management varies by plan.
Foundayo (orforglipron tablet)
- Kind
- FDA-approved GLP-1
- How it differs
- A non-peptide daily GLP-1 pill from Lilly for weight management, taken with or without food. No fasting window.
- Evidence
In the 72-week trial in adults with type 2 diabetes (Trial 2 on the label), weight fell 9.6% on 17.2 mg against 2.5% on placebo; without diabetes the figure was 11.1% against 2.1% (Foundayo label). Its label is for weight management, so it is not a direct replacement for diabetes treatment.
- Starting price
- $149 per 30 days at 0.8 mg (2.5 mg also $149 through 2026); regular price reaches $349 at 14.5 and 17.2 mg (Foundayo savings page, checked 5 Oct 2026)
- Best for
- Adults focused on weight who want a pill without the empty-stomach rule
- Tradeoffs
- No glycemic-control indication. Newest drug on this list. Higher doses cost more than the starting price.
Compounded semaglutide pill or ODT
- Kind
- compounded (not FDA-approved)
- How it differs
- Semaglutide made by a 503A compounding pharmacy as a tablet or dissolving tablet, often with an added ingredient. Not Rybelsus and not FDA-approved.
- Evidence
No trial has tested any compounded semaglutide tablet, so the PIONEER results on the Rybelsus label do not carry over. The FDA says compounded drugs are not reviewed for safety, effectiveness or quality before sale and should be used only when an FDA-approved drug cannot meet a patient's medical need (FDA, updated 1 Oct 2026). Temi calls it a separate, non-FDA-approved preparation.
- Starting price
- $85 for a 30-tablet pack (60 mg total) in California, with Temi's $39 fee on each order (Temi compounded semaglutide pill page, checked 5 Oct 2026)
- Best for
- Cash payers who have weighed the lack of FDA approval with a clinician and still want an oral option
- Tradeoffs
- No FDA approval, no label trials, and absorption of a compounded tablet is unstudied. Strengths do not map to Rybelsus or Ozempic tablet doses.
Which to choose
| If you | Consider | Why |
|---|---|---|
| Were doing well on Rybelsus and want the least disruption | Ozempic pill | Same molecule, same label and routine. Ask your prescriber: which Ozempic tablet strength matches my current Rybelsus dose? |
| Struggle with the empty-stomach morning routine | Ozempic pen or Mounjaro | Weekly injections have no fasting window. Mounjaro lowered HbA1c and weight more than semaglutide 1 mg in SURPASS-2. |
| Pay cash and need the lowest monthly cost | Metformin, dapagliflozin or generic liraglutide | All three are FDA-approved generics with cash prices from about $5 to $98 at Cost Plus Drugs, though none matches semaglutide's effect size. |
| Also have heart failure or chronic kidney disease | Dapagliflozin | Its label covers those conditions as well as blood sugar. Ask whether it should be added to, or replace, what you take now. |
| Took Rybelsus mainly to lose weight | Wegovy pill or Foundayo | Both carry a weight-management indication that Rybelsus never had, and both start at $149 a month self-pay. |
| Want an oral semaglutide outside the brand | Compounded semaglutide pill | It costs less, but it is not FDA-approved and its tablets have not been studied in trials. |
Switching
Moving off Rybelsus always means a new prescription, even to the Ozempic pill, because the strengths differ and pharmacies cannot swap them. The Illinois notice also says anyone who has just started Rybelsus 3 mg should not move to the Ozempic pill within the first 30 days. Bring your current strength, how long you have been on it and your latest HbA1c, and ask: what should I take, at what dose, and when do I start? If the prescriber is changing too, see how to switch GLP-1 providers without a gap in treatment.
Frequently asked questions
What replaced Rybelsus?
The Ozempic pill. NovoCare tells patients that Rybelsus is being replaced by the Ozempic pill, which shares Rybelsus's FDA label and active ingredient but comes in 1.5 mg, 4 mg and 9 mg tablets instead of 3, 7 and 14 mg. Illinois Medicaid reported that Rybelsus shipments to wholesalers stopped in May 2026. A new prescription is needed to move over.
What is a cheaper alternative to Rybelsus?
Generic metformin costs $5.32 for 30 tablets of 500 mg and generic dapagliflozin $7.43 for 30 tablets of 10 mg at Cost Plus Drugs, before shipping (checked 5 Oct 2026). Both are FDA-approved but work differently from semaglutide. The cheapest semaglutide tablet with FDA approval is the 1.5 mg Ozempic pill at $149 a month self-pay through NovoCare.
Is there a generic Rybelsus?
No. As of 5 October 2026, Drugs@FDA lists oral semaglutide only under Novo Nordisk's own application, with no approved generic. Compounded semaglutide tablets exist, but they are pharmacy-made and not FDA-approved, and they are not a generic of Rybelsus. The nearest generic GLP-1 is liraglutide, which is injected once a day.
Is there a diabetes pill like Rybelsus without the fasting rule?
Not among GLP-1 tablets labeled for diabetes. Both Rybelsus and the Ozempic pill need an empty stomach and a 30-minute wait (Rybelsus and Ozempic tablets label). Foundayo can be taken with or without food, but it is approved for weight management, not blood sugar control. Metformin and dapagliflozin are diabetes tablets without that rule, from different drug classes.
Can I use the Ozempic pill if my Rybelsus was for weight loss?
The Ozempic pill has the same label as Rybelsus, which covers type 2 diabetes, not weight management. The Wegovy pill is the oral semaglutide approved for weight, and Foundayo is a daily GLP-1 pill with the same indication. Ask your prescriber which matches your diagnosis, since that also affects what insurance will consider.
Does Temi prescribe Rybelsus or the Ozempic pill?
No. Temi has no product page for either and does not fill them. Its physicians review compounded semaglutide and tirzepatide requests (not FDA-approved products) and may prescribe Foundayo, dispensed by LillyDirect. For an existing metformin prescription, Temi reviews refills for $15 per refill review (usetemi.com/prescription/refills, checked 5 Oct 2026).
