The short answer: most people regain a large share of the weight they lost after stopping a GLP-1 medication, and most of it comes back within a year. A new small trial suggests that structured support after stopping, like a weight app or delivered meals, can slow that regain. It did not stop it.
What happens when you stop
Two large trials looked at this directly.
- In the STEP 1 extension, adults stopped semaglutide 2.4 mg (Wegovy) after 68 weeks. One year later, they had regained two-thirds of the weight they had lost.
- In SURMOUNT-4, everyone took tirzepatide (Zepbound) for 36 weeks. Then half were switched to a placebo without knowing it. Over the next 52 weeks, the placebo group regained 14% of their body weight. The group that stayed on tirzepatide lost another 5.5%.
The new study: support after stopping
A pilot randomized trial published October 7, 2026, in Obesity Science & Practice followed 39 adults for four months after they stopped a GLP-1. Researchers from Tufts University and UT Southwestern split them into three groups:
- Usual care
- A weight-management app for tracking food, exercise, and weight, used at least 15 minutes a day
- Home-delivered meals designed by dietitians, 10 a week chosen from a menu of 70
After four months, the usual-care group had regained about 10.75% of their body weight, Healthline reports. The app group regained 3.6%, and the meals group 3.58%.
What the study does and does not show
- It is small. 39 people. A pilot trial tests whether a bigger trial is worth running.
- It is short. Four months, against a year in STEP 1 and SURMOUNT-4.
- Support slowed regain. It did not prevent it. Both groups with support still gained weight back.
- Timing matters. The lead author told Newsweek she expects the most support is needed in the first year after stopping, when the risk of regain is highest.
If you are thinking about stopping
- Talk to your prescriber first. Ask whether to stop all at once or step down, and what to watch for afterward.
- Plan the first year before you stop. That is when most regain happened in these studies. Decide ahead of time how you will track your weight, what you will eat, and how you will stay active.
- Name the reason. If it is side effects, your prescriber may have other options. If it is cost, compare prices before you stop. Cash prices for the same medication vary widely between pharmacies and programs.
Where Temi fits
If you decide to stay on treatment, or to restart later, a Temi physician can review your request, and you choose the pharmacy after comparing prices in the open. See GLP-1 options.
