What is febuxostat?

Febuxostat is a selective xanthine oxidase inhibitor. It lowers uric acid in the blood and is prescribed long-term to manage gout and hyperuricemia in patients with gout. The FDA approved it in 2009 under the brand name Uloric.

Febuxostat carries an FDA boxed warning for increased risk of cardiovascular death compared with allopurinol.

The warning was added in 2019 after the CARES trial showed a higher rate of cardiovascular death in febuxostat patients versus allopurinol patients. This is especially relevant for patients with existing cardiovascular disease or stroke history. Discuss the risk-benefit balance with your prescribing physician, and seek care for chest pain, shortness of breath, rapid or irregular heartbeat, weakness on one side of the body, or sudden severe headache.

Other serious reactions

Febuxostat can cause hepatotoxicity (liver injury), and rare severe hypersensitivity reactions including Stevens-Johnson syndrome (SJS) and DRESS. Stop the medication and get care if you develop jaundice, dark urine, severe abdominal pain, a widespread rash, fever, or blistering.

Avoid combining febuxostat with azathioprine or mercaptopurine. The combination can cause life-threatening bone marrow toxicity.

Monitoring while taking febuxostat

Your physician typically checks liver function tests at baseline and periodically, along with serum uric acid to confirm the drug is reaching target. Patients with cardiovascular risk factors may need additional monitoring.
